Online consultations are currently available only for patients whom our doctors determine to be suitable for this service.
Thank you for your understanding.

1. Obtain a Password

Please contact our clinic to receive the password required for online consultation.
Access the questionnaire using the QR code or the URL below.

2. Registration of Personal Information

Online consultation requires your consent and registration of personal information.
Please enter your password to begin completing the form.

3. Payment & ID Registration

Please select your payment method (credit card or cash) and upload your My Number Card (health insurance card) or your Eligibility Certificate.

4. Complete the Questionnaire

Please fill out the questionnaire.
There is also a free comment field where you can describe your symptoms or concerns.

5. Receive Confirmation

After submitting the questionnaire, you will receive a confirmation email from our clinic.

6. Join Your Consultation

We will send you a video consultation link by email.
Please click the link at your scheduled appointment time to enter the virtual waiting room.
